ABSTRACT

ABNORMALITIES during pregnancy are regarded as pathological pregnancy. Early detection of anomalies is urgent to take measures as soon as possible to prevent serious pregnancy complications. Procoagulant-anticoagulant imbalance is often the pathological basis of many pathological pregnancy conditions, leading to a hypercoagulable state. We compared the thromboelastographic parameters between pathological pregnancy and normal pregnancy patients, evaluated the value of thromboelastography in early detecting pathological pregnancy and predicting postpartum hemorrhage.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of Chinese medicine (CM) and Western medicine (WM) on quality of life (QOL) after conservative surgery for endometriosis.</p><p><b>METHODS</b>A total of 320 patients with endometriosis were randomized into two groups by using random block design, CM group (160 cases, activating blood circulation and removing blood stasis treatment based on syndrome differentiation) and WM group (160 cases, gonadotropin-releasing hormone agonist or gestrinone treatment) after conservative surgery. Treatment was given for 3-6 months (according to the revised American Fertility Society scoring system stage), and the World Health Organization QOL-BREF (WHOQOL-BREF) was applied to patients before and after treatment to assess QOL.</p><p><b>RESULTS</b>There were 136 cases in the CM group and 141 cases in the WM group completing therapy. In the CM group, the use of the WHOQOL-BREF showed that the physical, psychological and environmental scores posttreatment were significantly higher than those at pre-treatment (P < 0.05), and for 12 items (pain and discomfort, energy and fatigue, sleep and rest, mobility, activities of daily living, work capacity, negative feelings, health and social care: accessibility and quality, participation in and opportunities for recreation/leisure activities, appetite, QOL score, overall health status and QOL), the difference in scores was significant (P < 0.05). In the WM group, 4 items (pain and discomfort, opportunities for acquiring new information and skills, QOL score, overall health status and QOL) had significantly different scores post-treatment compared with those at pre-treatment (P < 0.05). Before treatment, the QOL in the two groups of patients showed no significant difference (P > 0.05). After treatment, the scores for physical health in the CM group were significantly higher than those of the WM group (P < 0.05) and the scores of 4 items (mobility, activities of daily living, sexual activity, QOL score) in the CM group were significantly higher than those in the WM group (P < 0.05).</p><p><b>CONCLUSIONS</b>CM and WM treatment could improve the QOL of patients with endometriosis after conservative surgery. CM treatment is more effective than WM.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the clinical effect of Chinese medicine (CM) and Western medicine (WM) for controlling the recurrence of pelvic endometriosis after a conservative operation.</p><p><b>METHODS</b>The study was a multi-center, randomized, parallel controlled and prospective clinical trial. Patients were randomly divided into two groups: CM group (106 cases) and WM group (102 cases). Drugs were given to patients during 1-5 days of the first menstruation after a conservative operation in both groups. Patients with stages I and II (revised American Fertility Society) were treated for 3 months, while the patients with stages III and IV were treated for 6 months. The patients in the CM group were treated using three types of Chinese herbal medicine based on syndrome differentiation. Patients in the WM group were treated using gonadotropin releasing hormone agonist (GnRH-a) or gestrinone. Patients treated with GnRH-a received add-back therapy of Tibolone Tablets once a day after 4 months of treatment. Any cases of dysmenorrheal chronic pelvic pain, menstruation and any adverse reactions of patients were recorded once a month during the preoperative and postoperative periods and once every 3 months during the follow-up period. During the preoperative, postoperative and the follow-up periods, patients underwent type B ultrasonography of the pelvis and measurements of serum CA125 levels, gynecologic examination, routine evaluations of blood, urine, hepatic function (glutamate pyruvate transaminase), renal function (blood urea nitrogen) and electrocardiograms. During the follow-up period they underwent type B pelvic ultrasonography, measurement of serum CA125 levels and further gynecologic examinations. The two treatments were compared for clinical recurrence rates, pregnancy rates and the incidence of adverse reactions.</p><p><b>RESULTS</b>The incidence and timing of recurrence of endometriosis were not significantly different between the two groups. The first pregnancy achieved by the patient in the CM group was significantly earlier than that in the WM group (P <0.05). Moreover, the incidence of adverse reactions in the WM group was significantly higher than in the CM group (P <0.01).</p><p><b>CONCLUSIONS</b>Treatment with Chinese herbal medicines prevented the recurrence of endometriosis after a conservative operation, improved the conception rate and showed fewer and lighter adverse reactions than did treatment with WM therapy. Treatment with Chinese herbal medicine meets the need of patients wishing to have a child following endometriosis and is an appropriate form of clinical treatment.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the relationship of sperm DNA damage with unexplained recurrent spontaneous abortion (URSA).</p><p><b>METHODS</b>Sperm DNA fragmentation was evaluated by sperm chromatin dispersion (SCD), and the results were expressed in terms of DNA fragmentation index (DFI). DFIs were measured in the male partners of 56 women with URSA (the experiment group) and of 31 without URSA (the control group).</p><p><b>RESULTS</b>The DFI was shown to be (11.0% - 56.9%) in the experiment group, 21 (37.5%) of the subjects over 30%, as compared with (10.0% -36.8%) in the control group, only 8 (25.8%) of the subjects over 30%, significantly higher in the former than in the latter (29.4% vs 25.5%, P < 0.05).</p><p><b>CONCLUSION</b>There is a relationship between sperm DNA damage and URSA.</p>

ABSTRACT

HLA-G is a major histocompatibility complex [MHC], class Ib molecule that is selectively expressed at the fetal maternal interface. It is thought to play a role in protecting the fetus from the maternal immune response. Interestingly, the preimplantation embryo development [Ped] gene product Qa-2 is also a mouse MHC class Ib protein that affects cleavage and division of preimplantation mouse embryos and subsequent embryonic survival. Data from many human in vitro fertilization [IVF] clinics suggest that the mouse Ped phenomenon also exists in human because embryos fertilized at the same time have different cleavage rates and consequently different IVF outcomes. As HLA-G is expressed in human early embryos, it is highly regarded as the functional homologue of Qa-2. Whether HLA-G expression is correlated with the cleavage rate of human embryos has great potential clinical value. In this study, 45 human early abnormal fertilized embryos [3 PN] from patients undergoing in vitro fertilization were used to test the effects of HLA-G knock-down via infection with adenovirus carrying its specific siRNA on the cleavage rate in a 2-day culture period. One-way ANOVA, Post hoc and Chi-square were used to compare groups. A p-value smaller than 0.05 was considered statistically significant. Knocking-down HLA-G in human pre-implantation stage embryos resulted in a higher cell arrest rate and a slower cleavage rate. The results from the present study suggested that HLA-G might play an important role in early human embryo development

ABSTRACT

<p><b>OBJECTIVE</b>To study the advantage of fir bark splint for the treatmet of distal radius fracture.</p><p><b>METHODS</b>From January 2006 to June 2008, 80 cases were randomly divided into two groups and treated by fir bark splint fixation or plaster fixation. There were 40 cases in the fir bark splint group, including 18 males and 22 females with an average age of 45.2 years ranging from 20 to 66 years. There were 40 cases in the plaster group, including 19 males and 21 females with an average age of 44.1 years ranging from 23 to 65 years. Four weeks after manipulative reduction and external fixation, the fixtion effect, function recovery, bone mineral density of secondly metacarpus were obsearved by X-ray film, and the clinical effect were evaluated according to healing time ahead of schedule.</p><p><b>RESULTS</b>As compared between the two groups, the fixation effect of the fir bark splint group was better than that of the plaster group (Z = -2.844, P = 0.004 < 0.05); the function recovery of the fir bark splint group was better than that of the plaster group (Z = -3.196, P = 0.001 < 0.05); the bone mineral density of secondly metacarpus of the fir bark splint group were better than that of plaster group (t = 4.56, P < 0.001); the curative effect of fir bark splint group was better than that of the plaster group (Z = -2.601, P = 0.009 < 0.05).</p><p><b>CONCLUSION</b>Fir bark splint could effectively keep fixation stability and promote fracture healing as well as function recovery.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the clinical efficacy about manipulative reduction of the radialis and ulnar fractures.</p><p><b>METHODS</b>From June 2005 to June 2009, 85 patients with radialis and ulnar fractures, 50 males and 35 females, ranging in age from 1 to 20 years with an average of 10.8 years, were treated with manipulative reduction and external fixation by small splint. There were 53 cases of direct violence, 30 cases of transmission violence, 2 cases of reversing violence; and there were 6 cases of comminuted fracture, 8 cases of spiral fracture, 21 cases of oblique fracture, 50 cases of serrated or transverse fracture. The clinical effects were analyzed according to Anderson criterion.</p><p><b>RESULTS</b>Forty-two cases obtained anatomical reduction and 35 cases obtained nearly anatomical reduction and 8 cases occurred re-displacement. Eight cases of re-displacement were re-fixed after manipulative reduction, 2 cases obtained anatomical reduction and 4 cases obtained nearly anatomical reduction; 2 cases unsuccessfully were diverted to open reduction and internal fixation. All the fractures obtained clinically healing with average of 42 days (ranged from 28 to 80 days). All patients were followed up from 3 to 10 months with an average of 7.5 months. According to Anderson criterion, 77 case got excellent result, 5 good, 3 fair.</p><p><b>CONCLUSION</b>Manipulation and external fixation by small splint for the treatment of radialis and ulnar fractures, have advantage of less trauma, forceful fixation, quick healing, cheap cost, which can obtain satisfactorily clinical effects.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the process of eliminating tannin with polyamide-adsorption for preparing Xinshao lyophilyzation.</p><p><b>METHOD</b>The transfer rate of paeoniflorin, tannin examination, the test of the local stimulus and abnormal toxicity on mice were adopted as indexes to optimize the process of tannin adsorption and resolution of polyamide-adsorption, and the effect of eliminating tannin of polyamide-adsorption was compared with traditional methods.</p><p><b>RESULT</b>The effect of eliminating tannin with polyamide-adsorption was good, and the transfer rate of paeoniflorin with polyamide-adsorption was superior to the other methods. Furthermore, the abnormal toxicity decreased evidently and the security of the preparation was ensured.</p><p><b>CONCLUSION</b>The method of polyamide-adsorption provides a useful reference to the technique of eliminating tannin in the traditional Chinese medicine injections.</p>

ABSTRACT

Objective To construct the recombinant plasmid carrying shRNA to ATX and analyze the nucle- ic acid sequence for further searching new gene therapy method of tumor.Methods Two DNA sequences containing short hairpin structure were designed and synthesized.The complement form was obtained by annealing and inserted into vector Psilcncer2.1-U6 neo,and the recombinant plasmid was transformed into DH5a strain.Finally the plasmid identified by restriction enzyme was used for sequence analysis.Results The recombinant Psileneer2.1-U6 neo car- rying shRNA to ATX had been constructed and the aim sequence had been obtained.Conclusion The construction of the recombinant plasmid carrying shRNA to ATX lays the basis for the study of its inhibitive effect on tumor.
